What is a geriatric care manager?
A geriatric care manager is a professional who helps older adults and their families navigate the complexities of aging. We assess each individual's needs, create personalized care plans, coordinate services, advocate for our clients, and provide ongoing support to help families make informed decisions.
Who do you help?
We primarily help adult children who are caring for aging parents, as well as older adults who need guidance navigating healthcare, housing, memory care, and long-term planning. We also partner with elder law attorneys, hospitals, assisted living communities, and other professionals.
What services do you provide?
We offer comprehensive aging care management services, including care planning, hospital and medical advocacy, housing guidance, dementia support, crisis intervention, family communication, Medicare and Medicaid guidance, long-term care insurance assistance, referrals to trusted professionals, and ongoing care coordination.
Can you help families who live far away from their loved one?
Yes. Many of our clients are long-distance caregivers who need someone they can trust to be their eyes and ears. We provide regular updates, attend appointments, monitor changes, and help families make confident decisions from anywhere.
Can you attend medical appointments with my loved one?
Yes. We can accompany clients to physician appointments, emergency room visits, specialist consultations, and telehealth appointments to ensure questions are answered, concerns are addressed, and families stay informed.
Can you help with Medicare, Medicaid, and long-term care insurance?
Yes. We help families understand available options, connect them with qualified professionals when appropriate, assist with Medicaid guidance, and help initiate long-term care insurance claims.
